What actions cause for push-up disqualifications?

Greetings Rangers, Soldiers and Veterans. I've already used the search function but couldn't find it listed anywhere. The question I have is:

What actions will cause a disqualification during the APFT (regarding push-ups)?

I know improper posture (sagging back...not going low enough and etc) won't be counted as a proper military push-up. What happens if during the 2-minute time you reach failure and crash onto the floor....does that mean your time is up?

Are there any "allowed" resting positions (ie, locking elbows, arching back) before you continue on with push-ups?

I've started Ranger Silverback's posted routine and man....its a killer. :D I love it!!

Also bringing a hand or foot off the ground at any time during the event is cause for termination.

I guess that's all you'd really need...
