There is more to this story that you are not bringing up. They would not have randomly decidid to ELS you. What is the rest of the story?
If you think your back hurts now wear IBA and the rest of your kit 12 to 20 hours a day.....

The problem is there is more to the infantry than the APFT and weapons qual. If you can't bear a fighting load for a shit load of time, then you are of no use to us. Like rgrpuck alluded to, the IBA for an infantryman will create back issues where there once were none. If you have a medical problem from the get go, then do yourself a favor and get fixed. If the Army boots you for med problems then maybe you can be healed, and try again later. With an APFT score of 280 your problem is probably not fitness related, but all of us here have seen examples of soldiers whose weakest muscles were their hearts.
