I think a small amount of midazolam should be given to all patients. But, if you give a larger dose of ketamine, I do not think there is as much concern due to the sedation. I think the midlevel amounts cause the most problems.

If you carry everything that the CoTCCC carries you will be good. You can use 12G, it is just more difficult to push through the tissue, just make sure it is at least 3.25 inches in length.
